Care Navigation

When you call the Practice to make an appointment, the receptionist will ask for a brief outline of your issue to help them signpost you to the most suitable clinician. This is know as Care Navigation.

All reception Staff are trained to Care Navigate and will ask you some questions about the reason for your appointment. They will follow a protocol and WILL NOT MAKE CLINICAL DECISIONS.

All staff adhere to rules of confidentiality at all times.

Care Navigation offers individuals choice by providing information to help them to access the support that is right for them. This will help patients to see the right person, to provide the right care, in the right place, at the right time as efficiently as possible. This may be include consultation with:

  • Practice Nurse
  • Social worker
  • Mental Health Professional
  • Practice Based Pharmacist
  • GP

Cancel an Appointment

If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else. Patients who fail to attend three appointments in a 12 month period may be removed from the practice list.

Home Visits

Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling reception before 10:30am.

You may only request a home visit if you are housebound or too ill to visit the practice. Your GP will only visit you at home, if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls. All house call requests will be dealt with by the GP on call who will speak with you to assess the best response to your request.

You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.

Late For Your Appointment

Please attend your appointment on time, if you are late you may not be seen. If you are not seen you will not be able to rearrange your appointment until the next working day-except in the event of an medical emergency that requires immediate attention.

Interpreter

If you require an interpreter to attend with you when you see your Doctor, please notify the receptionist and they will arrange this for you.
